To begin services, please have the following available:
Psychological Evaluation
A report completed within the last 5 years that outlines your child’s diagnosis and needs.
Referral or Recommendation for ABA Therapy
This may come from a pediatrician, psychologist, or other qualified provider.
Individualized Education Program (IEP)
If your child has an IEP through their school, this can help us better understand their goals and support needs.
Insurance Card (Front and Back)
